Level 3 & 4 U.S. DOS Travel Advisory or CDC Level 3 or Excluded Coverage Destinations

Countries in this category are identified as those that may pose high threats to the University of North Texas faculty, staff, and students who travel to those countries for university-related purposes. In addition to registering travel in advance of departure, staff, students and their guests who are traveling for university-related purposes are required to complete the following steps to be considered for International Risk Oversight Committee, IROC, approval:

Faculty and Staff: Must complete and submit the Restricted Travel Exception Request Form. Travel must also be approved by the department's senior administrator to be considered for IROC approval.

Graduate Students: Must complete and submit the Restricted Travel Exception Request Form. Travel must also be supported by the department chair to be considered for IROC approval.

Undergraduate Students: Must complete and submit the Restricted Travel Exception Request Form. Travel must also be supported by the department chair to be considered for IROC approval.

*Guests, Dependents & Volunteers who are:

Accompanying a UNT Faculty/Staff Member: The guest, dependent, or volunteer must submit an electronic International Risk Acknowledgement Form and be listed on the faculty/staff’s approved Restricted Travel Exception Request Form, which must be approved by the International Risk Oversight Committee prior to accompanying a UNT faculty/staff member.

Accompanying a UNT Graduate Student: The guest, dependent, or volunteer must submit an electronic International Risk Acknowledgement Form and be listed on the student’s approved Restricted Travel Exception Request Form, which must be approved by the International Risk Oversight Committee prior to accompanying a UNT Graduate Student.

Accompanying a UNT Undergraduate Student: The guest, dependent, or volunteer must submit an electronic International Risk Acknowledgement Form and be listed on the student’s approved Restricted Travel Exception Request Form, which must be approved by the International Risk Oversight Committee prior to accompanying a UNT Undergraduate Student.

*A current UNT faculty/staff member or student cannot be considered a guest, dependent, or volunteer.
